Fall lawn care in Upstate New York?

I would like some advice as to what to do about my lawn so that it looks good in the spring. Right now the lawn has a lot of weeds and some bare spots. Basically, I need to grow new grass, feed what’s already there and prevent weeds in the spring but I don’t know when and what I should apply to do this. Please don’t recommend a lawn care company.
